Caregiver Support Sessions
Providing strengths-based psychosocial support for caregivers of people with Alzheimer's Disease/Dementia. Focus is on client-centred care to mitigate caregiver burden. Service delivered primarily via tele-health. Group or individual sessions may include counselling, education, strategies for challenging dementia behaviours, advance-care planning, complex family dynamics, mental health, grief and loss, facilitation of strengthening support networks, referrals to community resources, support to navigate health systems, strategies to implement respite, abuse/neglect issues, and transition to residential/complex care.
